The following health & wellness programs and resources are available for PreferredOne Members:

Health Risk Assessment (HRA)
Information is available online for PreferredOne member at
preferredone.com, select For Members, login and click Health Risk Assessment under Health Resources. Members can take this one-time free assessment and receive their own personal health score that will indicate their current and future health status.

PreferredOne Fitness Advantage Program
Offers PreferredOne members a credit of $20 towards the monthly membership at Life Time Fitness Clubs. Members must work out twelve days month to be eligible for the discount.

MidWest Employee Assistance Program (EAP)
Is available to PreferredOne members. EAP services include initial counseling for family, legal and debt issues. Information is available online at preferredone.com, select For Members, login and click Midwest EAP under Health Resources.

Health Resources
Information is available on the Web for PreferredOne members at preferredone.com, select For Members, login and check out the options under Health Resources. Featured are links to PreferredOne recommended health, wellness, medical and prescription drug websites that offer current information approved by the medical community.

PreferredOne Walking Program
Is based on the President's Council on Physical Fitness and is a guide on how to start a walking program and stay with it. The program can be used individually or as a group activity. Member Discount Program
Pair PreferredOne with verious organizations to offer discounts and services to help on the road to better health (e.g., Corporate Fitness Products, 2nd Wind Exercise Equipment, WeightWatchers Online, QualSight preferred LASIK pricing). Information is available online at preferredone.com, select For Members, login and click Member Discount Programs under Health Resources.

Disease Management for Rare and Chronic Diseases
Such as Multiple Sclerosis, Rheumatoid Arthritis, ALS and other such rare and serious diseases. Eligible members will be invited into the program by Accordant and receive specialized support and education customized for their condition.

Disease Management for Chronic Diseases Program
PreferredOne offers disease management for eligible members who are diagnosed with Chronic Heart Failure (CHF), Coronary Artery Disease (CAD), Chronic Obstructive Pulmonary Disease (COPD) and moderate to high-risk asthma. Members receive support and education from specialized case managers. All members with a co-morbidity of diabetes will have their diabetes managed along with their primary diagnosis.

Healthy Mom & Baby Program
Is a maternity management program for all expecting PreferredOne members. The main goal is early identificate of high-risk pregnancies. Eligible participants will be sent an introductory letter and self assessment that will help determine the risk for pregnancy complications. If the assessment indicates the possibility of a high risk pregnancy, participants will be directed to contact the PreferredOne Case Management Department for help and support. Participants will also receive a gift of "The Mayo Guide to A Healthy Pregnancy."
CareAssist NeoNatal Intensive Care Program
Is used in the event of premature or high-risk birth. This program links neonatologists to interact on behalf of the baby, parents and PreferredOne to ensure that appropriate care is taking place.

Medical Management
Is provided by an on-site deparment that assists members through the process of their medical care. Medical Management is staffed by RNs, LPNs and board certified physicians who review medical utilization, claims and physician reports to determine if a PreferredOne member needs to be in Case Management. These professionals focus on Pre-Certification, Prior Authorization, Concurrent Review, Post Service Review, Case Management and Appeals.

Case Management
Is available to all PreferredOne members when necessary
regardless of whether they are enrolled in a formal disease management program and is a standard part of PreferredOne's administrative services for fully insured plans. The goal is to cover any gaps in the care of member's chronic and acute conditions. Identification is made through diagnosis, high claim dollar amount and physician referral. Members are encouraged to respond to the Case Manager's invitation into case management.

Newborn Program
A letter is sent to each family of a newborn with information on well-baby visits, baby care and nutrition.

Men's Health Program
Targets all PreferredOne male members over the age of 49 with no record of a colonoscopy in the last nine years. A mailing is sent to each identified member reminding them to schedule a colonoscopy screening with their provider as well as getting regular screenings for high blood pressure (hypertension), cholesterol screening and prostate-specific antigen (PSA) test (prostate cancer).

Adolescent Immunization Program
Targets all PreferredOne members with children between the ages of 12 and 18. Each family received a letter listing the recommended immunizations, based on Centers for Disease Control data, needed for children in the 7th through 12th grads:

                Hepatitis B (Hep B)
                Measles, mumps, rubella (MMR)
                Polio
                Varicella (chickenpox)
                Tetanus, diphtheria, pertussis (whooping cough) (Tdap, Td)

Members are also encouraged to establish a relationship with a primary physician for their adolescent child.

Diabetes Program
Is available to all PreferredOne members diagnosed with diabetes and not enrolled in a formal diabetes management program. PreferredOne Case Management monitors these members and intervention is made only when it is apparent a member does not have their diabetes under control.

Migraine Headache Program
Is a program that directs identified members to a propert treatment plan, improving their quality of life, lowering work absenteeism, reducing the amount of emergency room and urgent care utilization, lowering plan and employer costs and encouraging them to find a primary care physician.
