Getting started
Tips on enrolling
To enroll in your employer's health plan, you must complete and submit an application available from your employer. To speed up the process, be sure to provide complete information, including your most recent health plan coverage.
After your completed application is processed, you will receive a member ID card and Certificate of Coverage.
Helpful forms and information
Your employer is required to provide you with some important information:
HIPAA Notice of Special Enrollment. You might decide to waive coverage for you, your spouse or a dependent at this time. This notice explains how you may request enrollment at a later date depending on your employer's plan.
General Notice of Preexisting Condition Exclusion. Small group plans offer guaranteed issue coverage. However, you and/or you dependents may be subject to preexisting condition limitations if you've had a lapse in medical coverage.
General Notice of COBRA Continuation Coverage Rights. Informs you about continuing your coverage if for some reason you are no longer eligible for you employer's plan. Ask your employer for these forms.

Talk to a Health Guide or Nurse Guide
When you have questions about your benefits, treatment plan, a health procedure or more, call the customer service number ont he back of your member ID card. You'll reach a Health Guide who will listen carefully to your concerns and quickly direct you to the right information, resources or support. You Health Guide may even connect you with one of our Nurse Guides to answer more in-depth health and medical questions.
Depend on a Dedicated Nurse when you face health challenges
If you or a family member is living with a serious health condition or have had a major illness or injury, you may be offered the services of a Dedicated Nurse. The Dedicated Nurse will call you at times to answer questions and help you understand your illness, medications and treatment options. Your nurse will also help you work with the health care system, set and reach goals, manage care from different sources, find community resources and more.
Usefull information at MyBlueCross member center
You have 24/7 online access to information about your health plan, claims, account details (if you have health accounts such as an FSA or HSA) and health improvement information. You access MyBlueCross by registering at members.bluecrossmn.com. Once you register, you simply sign in.

Steps to improve your health
Start with your Online Health Assessment
The Online Health Assessment can be your first step to living life well. Simply answer questions about your health history, nutrition, physical activity and lifestyle. It takes just 20 minutes and is confidential. Instantly, you get a report with results, health risks and health tips just for you. This $100 value is offered at no extra charge.*
You'll also find tools and resources to help you reach your health and wellness goals. There are free Online Coaching Modules that focus on fitness, nutrition, weight loss, reducing stress and other areas. Each module includes a personal hoe page, optional weekly e-mail reminders and your own action plan.
Find medical care
Provider information
With some of our plans, your care if coordinated through a primary care clinic. In others, you can select any doctor, clinic or hospital in your plan's large provider network. Check your Certificate of Coverage to learn which provider network you have. You can view doctors, hospitals and clinics in your network by signing in to myBlueCross at members.bluecrossmn.com. You'll also find information in your plan's provider directory, which you can request.
You're also covered wherever you work or travel in the United States through the BlueCard network of providers. You even have emergency coverage worldwide through the BlueCard Worldwide program. To find a participating provider, click on "Find a doctor" beneath Resources at bluecrossmn.com. Select the BlueCard national (or worldwide) network.
It pays to shop for care
The Car Comparison tool lets you compare facility-specific quality and cost (when available) for nearly 200 high-cost procedures and surgeries, such as MRIs, childbirth, tonsil removal, hip replacement, knee surgery and colonoscopies. You can compare facility quality measures, including the number of procedures performed, complication and mortality rates, average length of stay, patient safety and satisfaction.
At the Healthcare Scoop (thehealthcarescoop.com), you can read what people have to say about their actual experiences with doctors, hospitals and clinics.
Free visits to retail health clinics
If you have a copay plan, copays are waived at participating retail health clinics, such as MinuteClinic and Target Clinic. You don't need an appointment. Visits often take only 15 minutes. To find a retail health clinic near you, sign into myBlueCross at members.bluecrossmn.com. The clinics are part of your provider network.
Your prescription drug benefits
Save money on prescription drugs
Our plans include various levels of coverage for prescription drugs and discounted pricing at participating network pharmacies.
GenRx formulary helps you save
The new GenRx drug formulary (drug list) was designed to save you money. It offers a broad range of generic drugs. Plus, it includes bran-name drugs when generics are not available.
90-day supply for the price of two copays
With the 90dayRx program, you can get a 90-day supply of your refillable prescription drugs for only two copays (if you have a copay plan). You can use mail order and have your drugs delivered right to your door, or fil your 3-month prescription at a local participating pharmacy.
Free over-the-counter drugs
When members get a prescription for certain over-the-counter alternatives to some prescribed drugs, they pay nothing out-of-pocket. This program covers certain medications for stomach disorders and allergies. This program is now available for Options Blue HSA members (when their deductible is met).

Earn up to a $20 credit each month toward your

Having a baby? Get help
Healthy Start Prenatal Support is a personal, phone-based program for pregnant women. It's free to eligible members and is confidential. You'll have your own registered nurse trained in obstetrics who will answer your questions and share information. You'll receive the book Your Pregnancy Week by Week and other helpful materials, plus a $50 VISA gift card (for completing the program). To learn more, call the customer service number on the back of your member ID card.
Quit smoking for good
Take advantage of our free Stop-Smoking Support. It really works. Since the program began in 2000, more than 35,000 members have enrolled. On average, nearly one in three quit successfully. Call 1-888-662 BLUE (2583).
Call the 24-Hour Nurse Advice Line
When you have symptoms or health questions, call the Nurse Advice Line anytime, day or night. A nurse will quickly help you make treatment decisions. It's like having a medical advisor on call 24 hours a day. The toll-free number is 1-800-622-9524.
Help is a phone call away with EAP
When personal problems make life difficult, turn to the Employee Assistance Program (EAP). No problem is too big or too small. Counselors will listen to your concerns and help you take the next step. You and your family can get help with marriage and relationships, mental health, stress, family issues, legal referrals, childcare and elder care, money problems, sustance abuse and work issues.
